What should I bring to my first visit?
Your insurance card, a photo ID, any relevant imaging or physician notes, and your completed intake forms. Wear comfortable clothing you can move in.
How long will my visit take?
Plan for about 45–60 minutes for your first evaluation. Follow-up visits are typically shorter.
Where are you located, and is there parking?
We’re at 1224 8th St, Suite A in Rupert, right next to Minidoka Memorial Hospital. Free parking is available on site.
