New Patients
We’re excited to meet you! If this is your first time coming to our office, then we want to help you know what to expect. See our process below as well as our new patient forms. To save time on your first visit, download the forms below that apply to you, fill them out, and bring them with you to your appointment. If you have any questions, please don’t hesitate to contact us by phone or email. We want you to feel comfortable and know that your ease and happiness is important to us.
Arrival
Upon your arrival, you will be asked to fill out new patient information. You will need the information below:
- New Patient Forms
- Health Insurance Card
- Photo ID
Evaluation
On your first visit, we will review your medical history, muscle strength, flexibility, and any other relevant factors in relation to your injury or condition.
Treatment
After your evaluation, your Physical Therapist will create an individually-tailored treatment program and discuss how often you will need to attend physical therapy to achieve your long-term goals.
Follow Up
We will work to keep you on track to reach your long-term goals.

Insurance
Our practice accepts most insurances. If you have any questions about your insurance, please call.
Credit Card
We accept Discover, Visa, and MasterCard. A receipt is available upon request.
Cash
We can provide cash-based services for those who prefer this method or may not have insurance. Please call with any questions about this option.
Payment Plan
Payment plans are available upon request.
